Employees are always encouraged to initially discuss concerns with his or her supervisor. However, if an issue cannot be resolved or an employee believes there are instances of fraud, waste, abuse, mismanagement, misconduct, conflicts of interest, lack of compliance or other violations of federal law, state law, contractual agreements or University policy, there are several avenues to report your concern.

Internal Investigation Procedures
Employees are responsible for reporting any suspected wrongdoing or fraudulent activity. Reports can be made directly to the University Ethics Office or to the (OEIG).
Once complaints are received by the University Ethics Office, a determination will be made, based on the nature of the allegations, to determine the most appropriate office to address the concern. Additionally, complaints received by the University Ethics Office will be provided to the OEIG.
Any investigation conducted by the University Ethics Office will begin with the gathering of any needed documentation. Interviews will be conducted as needed. A determination will be made as to whether there is sufficient evidence to support the allegations, and any necessary communication with campus administration will follow.
The University Ethics Office takes all matters reported to our office seriously and reviews these concerns thoroughly. The Illinois State Officials and Employees Ethics Act (5 ILCS 430 20-95) specifically states that documents generated by an ethics officer are exempt from the provisions of the Illinois Freedom of Information Act (5 ILCS 140). Therefore, it is rare for the results of an internal review to be shared.
